Her department conducts the initial assessments and annual reassessments for Meals on Wheels and personal care. Susan outlines PAA’s mini geriatric assessment for every client, providing services as well as making referrals to other resources and services in the community. Susan is  also assigned to a number of income-based properties where seniors live independently but may need help with services in their homes to help them to continue to live independently.
